This whole school staff survey is designed to obtain staff views on implementation of PBIS. The survey examines the status and need for improvement of four behavior support systems: (a) school-wide discipline systems, (b) non-classroom management systems, (c) classroom management systems, and (d) systems for individual students engaging in chronic problem behaviors.


LEA PBIS Coordinators assist school based teams in deploying the survey to staff members via www.pbisassessment.org.


The website creates graphs to review survey results and also suggestions for creating an action plan.

Typically deployed within the first quarter of each school year to enable schools to make year to year data comparisons.

This survey is used to obtain staff perceptions to create or support a school’s PBIS action plan.

*Some schools/LEAs may deploy this survey at other times of the year, or multiple times of the year for progress monitoring.
